We are working with City policy leaders and local organizations to determine what COVID-19 reopening and long-term sustainability services are most needed for small merchants here in the Lower Polk. Your identity will be kept confidential. Your answers will help the small business community here in San Francisco.

All business types are encouraged to fill out the survey. This includes:

  • Restaurants
  • Gyms
  • Nail Salons
  • Stores of all types
  • Art Galleries
  • Performance Spaces
  • Repair shops of all types
  • And any and all other types of small businesses that have been impacted by COVID-19

If your business is NOT located in the Lower Polk area, please fill out the form anyway. If you are not a small merchant, please forward to someone who is.
